Information and Referral Specialist

JFS is seeking an Information and Referral Specialist to join our team.

JFS has been building community and enriching the lives of St. Louisans since 1871.  JFS provides children, adults and seniors in the St. Louis region with the supports they need to lead vibrant, healthy lives. Led by a team of dedicated and experienced therapists and staff, we offer comprehensive services, ranging from mental and behavioral health care to food and financial assistance.

The Information and Referral Specialist provides comprehensive case management to ensure that assigned clients have the tools and resources needed to support safe living at home and maintain social connections as they continue to navigate new stages of life. The Case Manager collaborates with clients to develop and manage individualized care plans that facilitate and support continued independence and safety in the client’s desired living environment and provides resources and program referrals, including subsidized in-home support services, falls and hospital re-admission prevention, and counseling and friendly visits as needed.

• Knowledge of community support services and resources for older adults
• Ability to counsel clients and/or families in life management and coping skills
• Ability to conduct effective interviews and psychosocial assessments
• Flexible, persistent and attentive to detail
• Skill in gathering data, compiling information and preparing reports
• Skill in preparing and maintaining accurate and timely case record documentation
• Skill in the use of office equipment, personal computers, and related software applications.

Requirements:

• Master’s degree in social work, counseling, psychology, or related field required.

Exempt, Full-time

Salary commensurate with experience
